The Alfred Hospital:
Alfred@Home
Founded
in 1871, The Alfred is Victoria’s oldest hospital operating on its original
site. The Alfred has a staff of 3500 and treats more than one quarter
of a million patients annually. It provides the most comprehensive range
of specialist medical and surgical services in Victoria and accommodates
six directorates comprising 42 clinical units, offering every form of
medical treatment with the exception of obstetrics and paediatrics.
The
Alfred @ Home program is a hospital in the home program, which aims to
provide individualised and holistic care to patients who require home
based acute nursing care. Alfred @ Home allows patients to receive ongoing
treatment in the familiar surroundings of their own home. Each patient
is individually assessed to determine his or her needs and a plan of care
is coordinated in consultation with a multidisciplinary team.
Alfred @ Home aims to:
- Provide continuous patient care from the hospital to the home.
- Minimise the disruption to the patient’s regular routine during their
treatment and empower the patient and their family.
- Minimise family separation and associated hospital.
- Coordinate your care from hospital to home and monitor your progress
- There is no cost to receive this hospital treatment at home
