(WHHO)
http://www.whho.org
The World Homecare and Hospice Organization (WHHO) is a dynamic network
of global leaders and organizations dedicated to the interests of all
patients, providers, home care associations, public and private sector
agencies, hospices, and personal services practitioners. Responding to
a worldwide need, key leaders in home care and hospice came together to
formulate and advance the concept of an international clearinghouse as
a forum for communication, business development, as well as information
and technology sharing. WHHO is the result of that vision.
WHHO, a culturally and socioeconomically diverse worldwide association
of providers and organizations involved in delivering health, hospice,
and social services, exponentially raises global awareness about and quality

Australia
Ambulatory Care Australia
http://www.health.vic.gov.au/aca/
Ambulatory Care Australia formerly known as The Victorian Centre for Ambulatory
Care Innovation (VCACI) was established in 1997, through a grant from
the Acute Health Division of the Victorian Department of Human Services.
In July 2002 the name was changed to Ambulatory Care Australia (ACA).
The mission of ACA is:
To support Australian health care providers as regional & international
health industry leaders in the development, trialing and evaluation of
ambulatory care initiatives involving the convergence of healthcare, technologies
and communications.
Home based acute care is a rapidly growing area of health care delivery
in Australia. There is a need to establish a local knowledge basis and
understanding of ambulatory care in Australia to inform changing clinical
practice.
The activities of ACA are underpinned by the belief that people are the
major healthcare resource and as such they require support and education
in health promotion and illness prevention.
ACA acts on the belief that people have a right to access models of care
which are patient centred, and to be able to make informed choices about
their health management and retain responsibility for their health status.
The shifting focus of health care delivery from hospitals to the community
has established a need for the development of expertise and greater understanding
in the area of ambulatory care.
ACA currently focuses on Hospital in the Home services and aims at providing
a research and evaluation framework for home based acute care.
